Karen McCarthy PT, DPT
Area DirectorKaren McCarthy received her bachelor’s degree in Health Science in 2009 and her Doctorate of Physical Therapy in 2012 from the University of Florida.
She began her career in the acute care setting while living in New Jersey. Working primarily in the ICU department, she successfully implemented protocols for early mobilization based on current research. After getting married, Karen moved to the Hudson Valley area and joined the Access Physical Therapy & Wellness family in early 2017 to further her career in outpatient orthopedics.
Karen’s clinical experience is primarily in orthopedic, vestibular, and geriatric physical therapy. She has a special interest in vestibular rehabilitation, and has completed continuing education coursework in that area. As a physical therapist, she loves making a difference in her patients’ lives and getting to know them on a personal level.
In her free time, Karen likes to relax and visit her hometown on the Jersey Shore.

DirectorNicole attended Springfield College, where she earned her Bachelor of Science in 2015 and her Doctorate of Physical Therapy in 2017. Since graduation, she has worked at Access Physical Therapy and Wellness for 3 years. She currently serves as the director of our Rhinebeck and Pleasant Valley offices.
She became certified in Graston Technique Levels 1 and 2 to help improve her patients’ soft tissue mobility and decrease their pain. Nicole is also certified in LSVT BIG, a program designed to improve the function and mobility of patients with Parkinson’s Disease and other neurological or balance impairments.
Nicole treats patients with a wide range of conditions including: orthopedic injuries, chronic pain, balance impairments, neurological conditions, vestibular impairments, pediatric injuries or torticollis, or post-surgical patients. She particularly enjoys working with each patient individually to help them attain their goals through use of evidence-based practice, manual therapy and individualized exercise plans.
In her free time, Nicole enjoys spending time outdoors walking her dog, coaching youth basketball, or visiting family and friends.

Physical Therapist AssistantAndrea Vignogna is a highly experienced Physical Therapist Assistant with 18 years of practice, having earned her degree from SUNY Orange. She holds Level 1 and Level 2 certifications in Graston Technique and has extensive expertise in treating a wide range of conditions, including orthopedic injuries, balance impairments, neurological disorders, and vestibular dysfunctions.
Andrea also enjoys working with pediatric and infant patients to address developmental delays, torticollis, and various orthopedic and neurological conditions. She is passionate about problem-solving and tailoring individualized treatment plans with a focus on various manual therapy skills and personalized exercise programs to meet each patient’s unique needs.
Outside of her professional work, Andrea enjoys hiking, spoiling her cats, and spending quality time with friends and family.
